Genetic Defects in Insulin Action
The causes of diabetes are unusual and result from Genetic Defects in Insulin Action. The metabolic abnormalities associated with mutations of the insulin receptor ranges from hyperinsulinemia and hyper-glycemia to severe symptomatic diabetes.
Two pediatric syndromes that have mutations in the insulin receptor gene with subsequent alterations in insulin receptor function and extreme insulin resistance are called leprechaunism and the Rabson-Mendenhall syndrome. Patients with insulin-resistant lipoatrophic diabetes alterations in the structure and function of the insulin receptor cannot be demonstrated. Therefore it is assumed that the lesions must reside in the post-receptor signal transduction pathways.
